Welcome to the Organization Diagnostic Questionnaire test page. The survey version of this page allows your organization to gather feedback on performance from its key stakeholders. THE ORGANIZATION DIAGNOSTIC SURVEY QUESTIONNAIRE INSTRUCTIONS: Below you will find several items know to impact organizational performance. You are asked to make two evaluations for each item. First you are asked to assess the "importance" of the item to the success of your organization. Then you are asked to assess the "performance" of the organization on that dimension. In both ratings you will be using a 5 point scale. The definitions for each scale are.. IMPORTANCE: 1 = NOT DESIRABLE, 2 = NOT IMPORTANT, 3 = NICE BUT NOT ESSENTIAL, 4 = IMPORTANT, 5 = TOP PRIORITY PERFORMANCE: 1 = URGENT NEED OF IMPROVEMENT , 2 = NEEDS IMPROVEMENT 3 = OK , 4 = GOOD, 5 = OUTSTANDING When you are finished, the "SUBMIT" button will send the questionnaire to us for processing.
