Make a purchase from the list below and click on the order button.You will be directed to a site where you can pay for the service and provide contact information.. Once we have confirmation that payment has been received, we will set up a unique survey under your organization's name. We will send you an access code which you can in turn supply to your respondents with an invitation to take your survey.Once the data is collected, we will supply you with another access code to view the results.
CUSTOMIZATION: Each of our generic surveys can also be customized. We can modify, add or delete questions. We can provide for sub-group identification with separate reports for each. And we can also customize the reports you receive. Customization will result in extra charges. You can contact us directly and we will give you a price or you can order the basic package and ask for customization and we will contact you.with a price.
